Case Details : A 49 years old male presented to casuality with 12 days history of high grade fever with cough with thick purulent expectoration . he was diagnosed as right upper lobe pneumonia with type II diabetes in another hospital and referred to us .At the time of admission pt was febrile with right ISA bronchial breathing with crepts . Investigations : Blood investigations showed leuckocytosis with deranged sugar levels .CT THORAX done confirmed the right upper lobe lung consolidation with cavity with centrilobular nodules over right middle and right upper lobe with atropic pancreas. BAL FLUID grownBURKHOLDERIA PSEUDOMALLEI. Treatment : Patient was empirically started on piperacillin/tazobactum with amikacin initially. initially sputum showed gram negative bacillinklebsiellia grown in culture and sputum afb was negative. Blood culture sterile. BAL samples showed BURKHOLDERIA PSEUDOMALLEI. Patient continues to have high grade intermittent fever. Hence patient has been switched on to meropenam and co trimoxazole and patient diabetic status has been managed with insulin .patient fever settled and general condition has been improved with treatment. Meropenam has been given for 14 days totally and patient has been discharged with maintainence dose of cotrimoxazole.
