This is a case report of a 25yr old lady, presented as primigravida at, 25 weeks and 4 days period of gestation with intrauterine demise of first twin for further management. The risk of mortality and morbidity in the surviving twin is considerable. Single foetal death in twin pregnancies was a tertiary referral centre, where intensive foetal surveillance and adequate neonatal support are available. The woman was admitted and following antenatal maternal and fetal surveillance, at 34 weeks of gestation, LSCS was performed and a preterm male baby of 1.5kg was extracted along with a dead macerated male fetus weighing 500g, with no visible anomalies. To date, follow up of the baby has been satisfactory.Conservative management with multidisciplinary approach and adequate monitoring can improve prognosis of the live fetus as well as decrease maternal complications.
