Does polyhydramnios have an impact on the maternal outcome?

Objectives: To study the maternal outcome in pregnancy with polyhydramnios and to compare the maternal outcomes in pregnancies with polyhydramnios to pregnancies with normal liquor. Methods: We compared the pregnancy outcome in 50 women with polyhydramnios i.e. AFI>24, to 50 women with AFI levels between 10 to 20. Women with gestational age >28 weeks were included in the study and were followed till delivery. This was a prospective observational study with no intention to treat. Results: Maximum numbers of cases as well as controls were in the age group of 21-35 years. There were 44% primigravida in the study group and 66% in control group. 60%, 22% and 8% of cases were in mild, moderate and severe polyhydramnios groups. 8 patients had amnioreduction. 5 patients were treated with indomethacin and 3 underwent amniocentesis. Maternal complications like PIH in 6 cases (P=0.749). PPROM in 7 cases (P=0.05). PROM was seen in 12 cases (P=0.06). Preterm labour in 9 cases (P=0.03). Malpresentation in 8 cases (P=0.04). 2 cases had cord prolapse. 11 patients (22%) had labour induced with Misoprostol, 7(14%) patients with Dinoprostone gel (PGE2) and17 patients (34%) went into spontaneous labour. In polyhydramnios group 64% had a vaginal delivery compared to 82% in control group. In polyhydramnios group, 4 patients had post-partum haemorrhage and one patient had placental abruption. In the control group, one patient each had post-partum haemorrhage, perineal tear and manual removal of placenta under general anaesthesia. Conclusion: Polyhydramnios has an impact on the maternal outcome in pregnancy.
