Abstract

Compendium of frequently asked questions at practical examinations about Abnormal Uterine bleeding emphasizing importance of menstrual history, mechanism of abnormal bleeding, treatment options with the rationale is provided. Abnormal uterine bleeding (AUB) due to ovarian dysfunction and/ or endometrial dysfunction may coexist with clinically detectable pelvic pathology.Apart from synchronized function of hypothalamo-pituitary-ovarian axis, vasoactive substances like prostaglandins, endothelins, and nitric oxide along with factors controlling endometrial breakdown and repair contribute to AUB. Persistence of the irregularity while on drugs or recurrence after attempts at the medical management is considered as treatment failure. Ablation techniques and use of LNG-IUS will help to postpone or avoid hysterectomy. Hysterectomy is considered for women who show treatment failure, have high risk endometrial hyperplasia, or those with history of hereditary cancer (eg., HNPCC, BRCA positive).
