Abstract
Objective: To validate efficacy of glucose tolerance test (GTT) in the diagnosis and treatment of gestational carbohydrate intolerance. Design: The prospective clinic-biochemical study involved 120 gravide. Based on postprandial sugar profile (PPS), 90 gravidae with abnormal GTT were divided into two groups: those with a) normal PPS getting no treatment (n=30); b) abnormal PPS on treatment and those receiving treatment based only on abnormal GTT (n=60). Setting: A teaching tertiary care hospital. Results: among the study groups, 20% women with abnormal GTT on treatment were found to have suboptimal glycaemic control. Higher incidence of neonatal hypoglycaemia (15 of 60 vs. 2 of 30) and hyperbilirubinemia (20 of 60 vs. 5 of 30) was noted among them. Incidence of heavy babies (>3.5 kg) born was also higher in treated group (11 of 60 vs. 3 of 30). The group with abnormal GTT but not treated since they had normal PPS were found to have comparable pregnancy outcome as in normal pregnant controls. Conclusion: All gravidae with abnormal GTT do not require treatment. Abnormal PPS should determine the need for treatment.
