Abstract

Background: Manual hyperinflation (MHI) helps in reducing complications of patients on mechanical ventilators by improving lung compliance. However, there are a variety of bagging units available and the efficacy of different types of bags in the improvement of compliance and changes in hemodynamic is not well-documented. Therefore, the objective of the present study is to compare changes in lung compliance and hemodynamic parameters produced by Mapleson C and Laerdal bag. Method: Design-Randomized crossover trial. Setting: Intensive care unit of Kasturba Hospital, India. Patients: Forty hemodynamically stable, intubated and ventilated patients, on SIMV or CMV mode of ventilation. Intervention: Patients were randomly assigned to receive MHI with either the Mapleson C or the Laerdal bag. Baseline measurement of the outcomes was done before, immediately after and thereafter at 30, 60 and 90 minutes after the treatment. After a washout period of about four hours, the sequence was reversed. Measurements: Blood pressure (BP), heart rate (HR), oxygen saturation (SpO2) and static compliance (Cs). Results: Significant improvement in Cs and SpO2 were seen with both circuits. These changes were not significantly different between the two techniques. Conclusion: Both techniques are equally effective in improving lung compliance. Hemodynamic responses are more favorable with the Mapleson C as compared to Laerdal bag.
